Sign Up for Mend
Grace now offers telemedicine appointments through a new platform, Mend. Funded by a grant from the Orange County government, this platform helps increase access to appointments with your provider. Please follow the steps below to set up your telemedicine appointment.
Your appointment with a Grace provider is scheduled. Your provider will determine if they would like to see you through the telemedicine platform. You must have an email address to create an account in Mend.
If you provider determines your appointment is suitable for telemedicine, you will receive a link to create an account in the Mend platform.
Once you create an account, you will be prompted to sign consent forms allowing communications between you and your provider using the Mend platform. Please accept all agreements in this prompt.
Next, you will receive a second link to sign forms related to your medical appointment.
Finally, you will receive a third link and code that you will use to access your medical appointment.
