OUR HISTORY

2010

Through the hard work of our trustees, Grace enrolled 1,200 patients and had 5,100 office visits in the first year of operation. At that time, the team consisted of 10 full-time staff members, 18 onsite medical specialists and 32 volunteer providers.

2011

The Medication Storage Room was created to allow Grace to provide patients access to medication they would otherwise not be able to afford.

2013

The UCF College of Medicine partnered with Grace to establish the Knights Clinic. Held twice a month at Grace, the clinic is coordinated by medical students from the UCF College of Medicine and staffed by students, faculty and physician volunteers under the guided supervision of licensed practitioners.

2014

Spiritual Care was added to address the spiritual health of patients. In 2014, Grace was also accredited by the National Committee for Quality Assurance as a level 2 Patient-Centered Medical Home (re-accredited in 2017 as a level 3).

2015

The first Worship Service was held, allowing the community to hear testimonials from patients about the impact of Grace.

2016

Grace began tracking “Social Determinants of Health” and treating the non-medical factors that impact health in the same manner as a medical referral (i.e., food insecurity, transportation barriers and job training skills).

2017

Through a clinical trial in partnership with Second Harvest Food Bank of Central Florida and Direct Relief, we began incorporating healthy food boxes in our Diabetes Prevention Program.

2019

Grace moved from a 7,000 to 19,000 square foot facility. The new location allows Grace to see more patients, expand existing services and add new services (including acute and preventative dental care) in response to the needs of the community.

2020

Since its inception, Grace has served more than 4,500 patients and helped local hospitals save millions in ER visits.

The team consists of 34 staff members, more than 25 onsite medical specialists and 493 volunteer providers.
