Empowering Communities: Grace Medical Home and Central Florida Black Nurses Association Join Forces for Health Equity

Grace Medical Home in partnership with the Central Florida Black Nurses Association  (CFBNA),  kicked off its first educational session under the prestigious Well Together Grant Awards Challenge in Eatonville last month.  As one of only five nonprofits selected by the Winter Park Health Foundation (WPHF) to receive this award, we’re honored to be part of their 30th-anniversary celebration and steadfast commitment to improving health and well-being in Eatonville, Maitland, and Winter Park.

This groundbreaking initiative brings us together with the Central Florida Black Nurses Association to tackle health disparities head-on. Our shared mission focuses on diabetes prevention and heart health improvement, two critical areas impacting our community. Through outreach, education, and screenings provided in Eatonville, we’re laying the foundation for a healthier future.

The first session offered a comprehensive overview of heart health, empowering attendees with practical knowledge on prevention and care. It’s the beginning of what promises to be a transformative journey for our community.
